Drug Alcohol Release

acknowledge that as a prerequisite of employment with PRS Pharmacy Services, I must provide a urine sample, as part of a drug and/or alcohol-screening test, to comply with the company’s Drug- and Alcohol-Free Workplace Policy and Procedure.

During the course of my employment, I,
hereby voluntarily agree to submit to any lawful drug and/or alcohol test requested and conducted by PRS Pharmacy Services, which PRS Pharmacy Services deems, in its sole discretion, to be reasonably necessary to provide its workers with a safe working environment.

I authorize that the results of any drug test be communicated and disclosed to the appropriate PRS clients, as required by their new hire standards. Consequently, any positive result obtained by said test, I understand that my job offer with PRS Pharmacy Services may be rescinded, or if currently employed by PRS Pharmacy Services, will be subject to disciplinary action, up to, and including, termination.

By proceeding, you expressly consent to receive marketing communications from PRS Pharmacy Services regarding Pharmacy Compliance, Ownership, and Education matters.
